California Healthy Kids Survey
5th and 6th graders will take the elementary surveys, and 7th graders will take the middle school survey. This survey is voluntary and requires parental consent. Consent forms will be sent home starting February 5th, and the survey will be administered February 26th through March 8th. Samples of the surveys are posted below.
